Acknowledgement of Risk, Waiver, and Release of Liability

A. I understand and acknowledge that participation in the camps, events, or programs operated or conducted by BeYoutiful Arts ('BYA”), including all of its activities and the use of its facilities and equipment, involves an inherent and unavoidable risk of injuries, harm, and loss. I understand that although BYA takes precautions to provide proper organization, supervision, and equipment, it is impossible for BYA to guarantee absolute safety. 

 I agree to indemnify, defend and hold BYA harmless in connection with any Claims arising out of my child’s/children’s participation in the camps, programs, events and activities operated by BYA. Additionally, if my student damages property they will be held accountable.

B. I hereby authorize the staff of BYA to act according to their best judgment in any situation requiring medical attention for the child/children named in this registration. I understand that it is my responsibility to provide medical insurance coverage for the child/children named in this registration while they are attending camps, classes or productions, operated by BYA and to provide accurate and complete medical information. I acknowledge that the costs of any medical treatment provided to the child/children named in this registration that are not covered by medical insurance will be my sole responsibility.

C. Social Media and Photography Release I give permission to BeYou Arts to photograph my child who is participating in a BA class/camp/program. I also give BA permission to use the photographs of my child for promotional purposes, including but not limited to the BA website, Newsletter, FB, Instagram and other marketing promo materials. 

I am aware that individual or group publicity photos or videos may be taken from time to time and in consideration for my or my child(ren)'s participation I hereby grant my permission for my child's name, image, & likeness to be used in print material, website, & social media. IF YOU DO NOT GRANT PERMISSION- PLEASE INDICATE SO IN THE "COMMENTS" SECTION OF YOUR REGISTRATION!

Waivers for A, B, & C become valid at point of registration.

Right to Terminate 

BYA reserves the right to terminate registration for a family or an individual parent/guardian in appropriate circumstances. It is possible to end our relationship with one parent/guardian, while designating the other as the sole contact point if the situation lends itself to this solution. 

 

Activities warranting termination of registration include, but are not limited to:  * failure or refusal to abide by camp policies or licensing requirements;  * failure or refusal to accept and cooperate in a camp decisions or practices;  * bullying, physical abuse, or verbal abuse toward staff, children, or other parents, including cursing or swearing;  * inappropriate, discourteous, or disrespectful interactions toward or communications with others, including on social media;  * demand for special treatment or services that are not provided to others, cannot reasonably be delivered by the program, or are outside the philosophy of the program. 

 

BYA Employees will attempt to discuss its concerns and allow a family the opportunity to correct inappropriate or disruptive conduct. However, the nature and/or severity of the conduct may require immediate termination of registration. 

 

Right to Terminate becomes valid at point of registration.

Refund Policy

f you are needing to cancel a registration, for any reason we will refund you in full all the way up until 20 calendar days prior to the start date of the event minus processing fees.
If canceled 19-14 days  to the start of the event, you will be refunded 50% of fees' minus processing fees
Death in the family can be refunded all but double processing fee, even within 14 days.  

There is no refund for Covid or Covid exposure. 
No refunds will be issued after 14 days to the start of  any program 

All tuition is non transferrable.  
 

Refund Policy becomes valid at point of registration.

2022 COVID-19 Liability Release Waiver for Clients

Due to theoutbreak of the novel Coronavirus (COVID-19), BEYOU ARTS is taking extra precautions with the care of every client to include health history review and enhanced sanitation/disinfecting procedures in compliance with CDC guidance.

I agree to the following:

• I have no symptoms and affirm that I, the current BEYOU ARTS STUDENT, as well as all household
members, do not currently have, nor have experienced any symptoms that are in line with COVID 19
within the last 7 days.
• I affirm that I, the current BEYOU ARTS STUDENT, as well as all household members, cannot be held liable for any exposure to the COVID-19 virus caused by misinformation on this form or the health history provided by each client.
